I think Thanos did it with the Infinity Gauntlet. Steve then attempted to use said gauntlet to fix it but created a worse problem that almost blew up Wakanda.

No, you're thinking of the Secret Wars event, marhawkman. Doctor Doom with the Beyonder's powers killed the Heroes with a bolt out of the blue, destroying the shield in the process. The end of the Secret Wars left "wishing" energy permeating Battleworld, and Cap used it to reform his shield.
Many, many years later, however, it was revealed that Cap did an imperfect job, and a microscopic flaw in the reformed shield caused it to shatter. What's more, this flaw caused the Vibranium in the shield to vibrate differently at a molecular level, creating a vibration that carried around the world affecting other forms of Vibranium as well, causing them to detonate as well. Cap finally used some pseudo-science machine to reform his shield and end the "Vibranium Cancer".
